• Immunization Requirements for Enrollment

     

    North Carolina General Statutes (G.S. 130A-152a) require immunizations for every child present in the state. Every parent, guardian or person in loco parentis is responsible for ensuring that their child(ren) receive required immunizations. A copy of the immunizations is to be presented when the child is registered for Kindergarten or when the child transers into a North Carolina school 

     

    Effective July 1, 2015, the administrative rule 10A NCAC 41A has been changed to add new requirements at kindergarten and 7th grade entry.

    KINDERGARTEN ENTRY

     

    DTP/DTAP - Diphtheria, tetanus, pertussis (5 doses)

    Second booster is required on/after the fourth birthday and before enrolling K-1 for the first time; if a fourth dose of DTP/DTAP was administered on/after the fourth birthday, a fifth dose is not required.

     

    Polio (4 doses)

    Booster (4th dose) is required on/after the fourth birthday and before entering school for the first time

     

    Rubella (1 dose)

    One dose on/after 12 months of age

     

    Hib-Haemophilus influenzae B (4 doses)

    Vaccine is not required if the child has passed the fifth birthday

     

    Mumps (2 doses)

    One dose on/after age 12 month; students entering school for the first time on/after July 1, 2008 are required to have a second mumps vaccine

     

    Hepatitis B (3 doses)

    Students born before 7/01/94 are not required to have the Hepatitis B Vaccine. If born on/after 7/01/94, 3 doses are required.

     

    Measles (2 doses)

    One dose on/after age 12 months, second dose at least 28 days later and before enrolling in K-1 for the first time after 1/01/94

     

    Varicella - Chicken Pox (2 doses)

    Doses must be administered at least 28 days apart, second dose must be given before entering school for the first time. Documentation of disease must be from a physician or physician's assistant verifying history of varicella disease. Documentation must include name of individual with history of disease, approximate date or age of infection, and healthcare provider's signature.

     

     

    7TH GRADE ENTRY

     

    Tdap - Tetanus, Diptheria, and Pertussis (Whooping Cough)

    Booster dose of Tdap for individuals who have not previously received it and are entering 7th grade or by 12 years of age, whichever comes first.

     

    MCV - Meningococcal Conjugate Vacine (2 doses)

    One dose for individuals entering 7th grade or by 12 years of age, whichever comes first. Booster dose for individuals entering 12th grade or 17 years of age beginning August 1, 2020. If the first dose is administered after the 16th birthday, the booster is not required.
