Dear Parent/Guardian:
Brunswick County Health Department will be coming to your child's school to offer in-school vaccinations for 6th and 11th grade students. This is an early opportunity for you to take care of
next year's school immunization requirement (lOA NCAC 41A .0401).
• Tdap and Meningococcal vaccine are both required for students entering the 7th grade
• A Meningococcal vaccine booster is required for students entering the 12th grade
*Check with your child's health care provider to see if your child has already received Tdap or
Meningococcal vaccine.
Brunswick County Health Department will administer vaccines ONLY to students who have signed
permission forms.
Information for vaccination dates, locations, and the permission forms will be posted on the
Brunswick County Health Services Immunization webpage and on Brunswick County Schools'
website.
*You must complete all sections on the form. If the form is not complete, we will not be able to
provide the vaccines to your child.
If you would like your child to participate in the school vaccination clinic, return the permission
form to your child's school as soon as possible.
Usually, individuals with insurance will not have any out-of-pocket cost. We will file your
insurance. The parent/guardian will be responsible for any costs not covered by insurance.
Students with no insurance or coverage for vaccines can qualify for state supplied vaccine at no
cost. You must fill out the insurance section on the permission form. For billing questions please
